What is the NHS Health Check?

The NHS Health Check has been designed to help you to lower your risk of developing four common but preventable diseases including heart disease, stroke, kidney disease and type two diabetes. If you are between the ages of 40-74 and have not been diagnosed with any of these conditions, you are eligible to receive a free NHS Health Check.

What are the benefits of having the NHS Health Check?

The NHS Health Check is designed to assess your risk of developing heart disease, stroke, kidney disease and diabetes. Once you've had your health check, you will receive personalised advice and support to help you to improve your lifestyle and lower your risk of developing these diseases.

The LiveWell team are working closely with the NHS Health Check programme to ensure that you are fully supported after receiving your health check. Once you have had your health check, you are encouraged to make an appointment with a LiveWell Health Advisor. Your Health Advisor will help you to set health-related goals that will support you in making changes to your lifestyle to improve your health.

Where do I access the NHS Health Check?

The NHS Health Check will be available in most GP Practices across Sutton and Merton. Please contact your local GP for further information.

How can LiveWell support me after my NHS Health Check?

LiveWell is working closely with the NHS Health Check programme. Following on from your NHS Health Check, you are encouraged to make an appointment with a LiveWell Health Advisor. Your LiveWell Health Advisor will provide you with additional support and information to help you to improve your lifestyle and work towards lowering your risk of developing disease.

If you have already had your NHS Health Check, congratulations! It is your first step to reducing your risk of developing heart disease, stroke, diabetes and kidney disease. The next step is to set some goals that could improve your health and keep well. Make an appointment with LiveWell Sutton and Merton for help in achieving your goals.
